Add character counters for constrained text fields

Problem

Description fields have length constraints in validation, but users do not see remaining characters while typing.

Suggested fix

Add counters to description and review comment fields.

Acceptance criteria

• Counters show current and max length.
• Counters update live.
• Error state appears before or at submission when limits are exceeded.
