Deliverable

• A qualtrics based intake form

Tasks

• Meet with Ceilyn and Danny to review intake questions and format
• Create a draft intake form based on feedback from the meeting in step 1
• Have UNC and NYU test the intake form
• Revise based on testing
• Retest
• Revise and finalize
