Server process does not terminate - extension?

This may not be an issue with the extension; if it is, it's presumably the worker thread that causes it.

When shut down, the process is still around, needing to be killed - usually manually.
If there are several servers running, copying/renaming the .exe used will help distinguish it from the others.

Perhaps check the count of thread_attach vs thread_detach?

Current state of this issue:

• a3 issue linked above shows this happening in another case where a thread is still running.
• the extension uses process_detach as a signal to shut down the db thread.
• The loader always closes properly. In this case, dllMain is called with PROCESS_DETACH.
• Tests show arma 3 remains in the background if the extension was loaded, but closes properly when the extension is not present. Dllmain is never called with process_detach.

This issue depends on BI implementing some shutdown signal, for example unloadlibrary on the extensions as part of shutting down.
