[Bug] Conversation title rename not saving - reverts after refresh

Bug Description

Conversation title rename does not persist after page refresh. The UI shows "Save successful" but the change is not saved to the backend.

Steps to Reproduce

1. Open an existing conversation in OpenHands Cloud
2. Click the rename button next to the conversation title
3. Enter a new title and save
4. UI shows success message
5. Refresh the page
6. The title reverts to the original value

Expected Behavior

Title should persist after page refresh

Actual Behavior

Title reverts to original value after refresh

• API PATCH request to update title also does not persist the change
