Allow for a link to the source blog, accompanying the feed listing in Subscribed Feeds #1219

The "Subscribed Feeds" page (https://digitalhumanitiesnow.org/subscribed-feeds/) currently displays a list of RSS feeds, and each feed listing is linked to the RSS subscription code. For readers of the DHNow site, it would be useful to be able to click through directly to the blog itself (as well as being able to get RSS feed information), since the Subscribed Feeds list is a valuable source of information on essential DH blogs and sources.

Ideally, the entry for each feed would display the name of the feed, a link to the subscription code, a link to the blog/resource itself, and a short description. (Support for a description field is proposed in issue #1209.) A possible way to organize/display this information would be:

Name of feed (RSS): description
The name of feed would carry the link to the source blog, and the word "RSS" would carry the link to the RSS code.
