Missing local login form

Upgraded plugin to latest version and I have no login form when going to /wp-login.php?normal=1.

I only see the WP logo, but not the form. Saml login is working correctly.

Duplicate of #129 and fixed by #135.
Just needs someone to merge #135 and make a release.
