As a user, I want to have a description or reason box in the Enable/Disable Rules modal within the Scan Results, so that I can document why a particular rule has been disabled. This will help in understanding the context behind disabling rules and improve the overall maintainability of the codebase.

Acceptance Criteria

1. A description or reason box is added to the Enable/Disable Rules modal.
2. Users can input text into the description or reason box when disabling a rule.
3. The reason for disabling a rule is saved and displayed when the modal is reopened.
4. The reason for disabling a rule is associated with the specific site and rule.
