Split CA DMHC External Appeal Entries on Physician #1
Description
A small subset (~500) of the CA DMHC reviews contain 3 independent physician reviews that have not yet been split. In these cases, they are delimited by Physician 1, Physician 2 and Physician 3 markers.
We should split these into independent entries, as we have for all analogous reviewer delimiters.
