Refactor the API into a more maintainable structure.

Requirements

• Move task routes into routes/
• Create a service layer
• Move SQL/database logic out of server.ts
• Keep route handlers thin
• Update documentation if needed

Deliverable

A cleaner API architecture with separation of concerns.
