Roundabouts generated crossing / isolated inner ring

When I generate the crossing for this case, I get 3 on each junction, which is 🟢 right.

But when I then generate the crossings (as ways), only one of the three gets generated 🔴 .

We talked about this topic in relation to #47 but since the crossings where generated, I think this is something else?

Imagine the red lines actually are straight / perpendicular 😅 . We're getting unlucky here with the angles; the perpendicular line projected from the crossing node doesn't hit a sidewalk on one side. We're using a fixed distance right now for the crossing nodes... it might work just to increase this a bit, though that's still brittle. Trying.

This case is fixed in a simple way, but it reveals more problems:

We really need to handle the angle of roads better, or look for the "corner" in the generated sidewalks to place things better. (This might introduce more dependency of bulk operation steps, because then we have to generate sidewalks before crossing nodes.)
