Mobile update Search Filter modal

This should only take the filters available currently on desktop. any multi selects will come after v1

1. Add in "Health Worker Status" as a single-select dropdown
2. Update the order of the fields per the mock
3. Update the header text to say "Update Search Criteria" (unlike the mock)
4. Update the main button at the bottom to say "Search"
5. Add a checkbox above the search button that says "Update Profile upon Search"
6. Symptoms and Conditions should be multi-selects instead of single-selects

Acceptance Criteria

• Complete the requests in the story above
• Follow the mock
