Emergency Notice

As a user, I want to be warned that this is not an application to be used if I may need emergency medical attention.

On the Get Started page immediately after the splash loading screen, show an alert that says
"If this is an emergency, call 911."

Note, per conversation: Waiting on legal to decide if this is necessary or not. Will not move forward, but if we do we will likely use a banner at the bottom instead of a modal
