Test other OIDC providers

Currently only Cloudflare Access has been tested as an OIDC IdP.

Others to test:

• Microsoft Entra
  • Interactive auth
  • Refresh token auth
  • Certificate issuance from CA
• Auth0
  • Interactive auth
  • Refresh token auth
  • Certificate issuance from CA

Basic auth flow with Auth0 works as of #34 being merged.

Auth via the refresh token did not seem to work so this requires further investigation and authentication token verification by the SSH CA Worker has not been completed.
