Failed to send confirmation email #17

The user was able to register an account using JOSSO1 package. I can confirmed that the user information is added to the system database. But the user never received the email for activation. So the account was never not activated.
The user email address is valid. We can confirmed that by sending test email through other email server.
From the JOSSO server, we also confirmed the email domain is reachable.
Please give us any debug hint.
