Design: Limit ability to add more than 5 organizational contacts with domain requests

Issue description

In the Domain Requests form, limit the number of organization contacts to a maximum of 5 contacts, that can be included under the Other employees from your organization section.

Once 5 contacts have been added, an error message should notify the user that they cannot add additional contacts.

Acceptance criteria

NEW DOMAIN REQUESTS

• Limit additional contacts to 5 entries.
• If user selects "yes" to input contacts, display new instructional text below that field that reads:
  • You may add up to five additional contacts.
• If user exceeds five contacts, display error message that reads:
  • Domain requests must have no more than 5 additional contacts. Remove one before adding another.
