Hm, an interesting problem! I have tested on Mac (case-insensitive underlying FS) so this hasn't come up. I guess we might have to do some sort of fuzzy matching when looking up paths -- I can't think of a better way than listing all files and attempting to case-convert when constructing paths, yuck.
