feat(fix): add action hints to fix agent comments

Summary

Same pattern as #1789 but for fix/code agent comments. After the fix agent posts its results, users don't know how to iterate or trigger a re-review. Adding a "Next steps" footer makes the workflow discoverable.

Proposed behavior

When the fix agent posts its outcome comment, include a footer:

- `/fs-review` — request a re-review of the changes
- `/fs-fix <instruction>` — run another fix pass with specific guidance
- Push commits directly — review re-runs automatically on push

Why

Same discoverability gap as #1789 — after receiving fix results, users don't know /fs-review exists or that pushing triggers re-review automatically.

• /fs-fix <instruction> — run another fix pass
• Push commits directly — review re-runs automatically

Proposed Test Case

- Verify the fix agent outcome comment includes the action-hint footer
- Verify the footer contains /fs-review and /fs-fix slash commands
- Ensure the footer appears only on outcome comments, not status updates
- Edge case: footer renders correctly on long outcome comments
