Sometimes streams don't get closed and streaming doesn't work for the broken user anymore

Happens when ingest signals stream start, and then the connection between ingest and qwer is lost, and upon reconnecting the already existing stream is added again to the "active" streams table.

Easiest solution is probably to rework stream_session to not own bandwidth_usage samples and not have a stop_time as well.
