Headers and Footers not working

When adding Headers and / or Footers to a file and enable the Ophir::HEADER and Ophir::FOOTNOTE option, i should see headers and footers, shouldn't I?

In fact Ophir::HEADER should be renamed to Ophir::HEADINGS. My mistake.
Footnotes are not the same thing as footers.

We could add support for headers and footers, but we need to think about how to do it, as hearders and footers do not make much sense in HTML.

In my personal opinion, i don't see the point in adding support for it. However since this is a project for converting this type of files, it would be a step towards converting every element of the document.

Maybe we could render it as slightly grey text in <small> tags but disable it by default? Something like in this screenshot: Seperate pages with a <hr> and add the headers and footers as <small>
