Separate Entity Model definition into separate file

# Current status
The entity model will be partially separated into ```entity_model.h``` and away from ```descriptors.h``` with #41. 

# Goal
Provide an separate entity model with which dynamic parameters like number of stream_inputs/stream_outputs can be assigned easily and in one place.
