Non-terminatable Executions

Sometimes, no matter what is the consistency, the execution never ends because some clients always get timeout, and are unable to execute even one simple transaction.

Preliminary assumption is that fractal sometimes is unable to init completely, thus does not deliver the amcast messages.

One reason could have been that Fractal generated the same id for different clients. This isse was fixed in Fractal. After that, this problem has not been observed.
