Breaking changes not being major bumped

• We need to update the PR template with a reminder that if you've made a breaking change (if someone pulls the latest version it may longer work) you need to follow the conventional convention for recording it, so that it gets picked up by semantic release. See an example BREAKING CHANGE commit: 63c3b75
• This may also require a Slack/engineering all hands reminder.
