Governed Goals for Real Multi-Project Workspaces

This release turns plain-language Goals into a stable, canonical-first workflow
across adopted projects, project sets, and complete workspaces.

What's New:

• Goal scope follows invocation context: an adopted project binds
  automatically, while a multi-project workspace asks for one project,
  multiple projects, or the entire workspace.
• Non-interactive and JSON consumers receive deterministic
  needs-confirmation decisions instead of prompts or guessed scope.
• Coverage Goals reuse canonical Workspace Model runtimes. A polyglot project
  can bind an explicit --runtime, and a multi-project Goal exposes only
  runtimes shared by every selected project.
• Mixed-runtime coverage scopes with no common runtime fail closed and request
  runtime-compatible Goal scopes; no project is silently omitted from
  verification.
• The VS Code consumer receives synchronized Goal, lifecycle, handoff, and
  verified-goal contracts for native project/runtime selection.

Reliability improvements:

• Git-backed Graph freshness now excludes managed .workspai outputs from the
  source inventory, preventing Workspace Intelligence and Goal --refresh
  from invalidating their own freshly generated evidence.
• Goal renewal, verification, Repair Engine boundaries, and project entry
  recovery preserve project-set scope and canonical runtime bindings.

Breaking changes: None. New scope values and measurement runtime choices
are additive; existing Goal artifacts remain readable.
