Add Claude Code plugin (MCP server + hooks)

obsxa has an OpenCode plugin but no Claude Code integration. Claude Code supports MCP servers for active tools and shell hooks for passive collection.

Two layers needed:

• MCP server exposing all 7 obsxa tool groups (observation, relation, cluster, search, analysis, promote, dedup)
• Hook handler script for PostToolUse, SessionStart, Stop events with dedup and skip-list
