Revolut: CSV has changed and script doesn't work anymore

@inobrevi — root cause found and fix is up as PR #67. Three problems stacked on top of each other:

1. UTF-8 BOM at the start of your file (Revolut's exports include it; our parser wasn't stripping it)
2. Column name case change (Date → date) which broke row['Date']
3. Unknown operation types (CASH WITHDRAWAL, DEPOSIT, TRANSFER) which were silently dropped by the old error handling
