Allow adding notes with user input in QnA tool

Codex allows us to add notes along with the selected option in the QnA tool user input prompt by pressing tab. T3 Code doesn't support adding notes with selection. It would be a helpful feature.

Thanks to @pzauner, who brought this to my attention first #332 (comment). Then, I experienced the need for this myself while using T3 Code in planning mode.

funny enough though: It's also not supported in Codex GUI, only TUI.
