Do not update signs in certain condition

Do not update signs if:

1. buffer is modified but not saved
2. coverage.json is not changed.

the signs update if you create a new coverage report file
the plugin reads from a json which contains file name ander line number etc
to show it in vim if you do not retest with coverage then it won't update
