The paragraph numbering code was fixed in ptxversion=7. Before this point, the numbering was frequently unpredictable. The old behaviour was preserved so as to not waste the work that some people had done in figuring out what paragraph number the code assigned to the one wanted to apply the update do.
The current right-click method assumes that paragraphs are numbered properly, which is incompatible with ptxversion<7. People who try to adjust the layout of a layout-preserved file should probably be warned.
This also suggests that some people might want different ptxversions for different books....
