Add ODK forms and translations to replace HOLPA content

Notes on translations:

• Full translations already exist for French, Spanish, English, Nepali
• This is nearly all of the ones we will need - only Haitian Creole and at least one of the Indian languages to add. But that is 1 partner for each of those
