Form submission and Include screenshot - functionality needs fixing

While reporting a bug, if you click on 'Include Screenshot', it opens another window/pop-up giving two options - Cancel and Allow.
If you click on Allow, it takes the screenshot of the page, and includes the image in the bug-detail.
But if you click on Cancel, ideally it should go back to Bug-filing window without the include screenshot image and the corresponding option should be 'un-ticked'. But, currently, it simply cancels entire bug filing window.
To return to the same bug filing, you have to click on 'Report a Bug' again, then your previous entries will be seen pre-filled, along with 'ticked' Include Screenshot but with No screenshot/image included.

Modifications required -

1. The form should clear itself out, and should not be pre-filled as current flow.
2. If the include screenshot is clicked, and there is no image attached, the bug should say 'Please include screenshot' before getting submitted.
3. The 'Bug reported Successfully' toast should appear at the center of the page instead of bottom-right of the screen.
