When an app update recreates a shortcut on the desktop, the old copy remains in the organizer directory. Implement RemoveDesktopDuplicateOrganizerReferences to detect items that already exist on the actual Windows desktop and remove the stale organizer copy automatically. This runs on startup, desktop page refresh, and organizer widget refresh.
