fix(review): make live publishing a secondary confirmed action

Product outcome

Keep Review centered on editing, save, copy/open, and export. Direct connector publishing becomes a separate secondary action with explicit account and revision confirmation.

Included

• pure direct-publish guard covering stale, failed, needs-review, unapproved, over-limit, permission, connection, expiry, and manual-only states;
• account identity and exact draft-revision confirmation contract;
• API truth helper that accepts success only for a matching platform with a stable post ID or URL;
• responsive secondary publishing panel and keyboard/Escape-accessible confirmation dialog;
• primary copy/open and save actions remain available independently of connector readiness;
• deterministic guard, confirmation, response-fixture, and UI-source tests.

Verification

Requires the complete frontend regression suite, production dependency audit, Next.js build, MCP tests, Python tests, and Vercel preview to pass before merge.
