Scope

Adds the token-gated ADD/DEL/PATCH fault controller and the integration configuration used by migration E2E.

Draft dependency caveat

This PR is opened early for review and CI on top of the temporary multi-parent quality join. Do not merge until the quality join and all inherited J1/Q1/Q3 prerequisites merge.

Behavior

No public enable flag is added. The endpoint is registered only when a non-empty out-of-band test token is configured, and authentication uses constant-time comparison.

Evidence

Linux/Windows lint, fault-path race tests, and integration configuration tests pass.
