feat: add simultaneous tick execution

Summary

• replace sequential model turns with frozen-observation simultaneous ticks, bounded concurrent dispatch, deterministic phased resolution, shared deadlines, cancellation, and lost-tick records
• add schema-v10 tick attribution, export telemetry, SQLite archive migration/query support, and production World Lab tick controls
• advance the agent decision contract to text-flat-json-v4 with selective communication guidance that discourages routine action narration while preserving channel and engine authority
• document tick authority, security, testing, archive behavior, and selective communication in ADRs 0014 and 0015

Known follow-up
