Improvements for readability of BookXML

BookXML should be fairly human-readable, as we do further editing/proofreading on it.

• Keep the spacing as per the samples we provided (essentially, block elements start a new line, inline elements are run in)
• As in the samples, common characters (quotation marks, apostrophes, dashes) should appear as those characters, rather than codes.

I have fixed the character entities part of this ticket. The whitespace part is still to be done.

Whitespace fixes are looking good overall. I see a few errors.

• Space around italic words (probably any <emph>) is being lost:

Spaces around <emph> tags look good.

Footnote reference fix is now putting a space before AND after the footnote refs. We want one ONLY AFTER the <fnoteref>.
