The Best Possible Medication History (BPMH) provides an accurate and up-to-date list of a patient’s prescription medications by compiling data directly from the Digital Health Drug Repository (DHDR). This list includes recent dispensations from Ontario pharmacies and serves as a foundation for safe and informed clinical care.
Medication reconciliation allows clinicians to download a patient’s medication data from the DHDR and manually review it for accuracy. By comparing the DHDR record against their clinical knowledge and the patient's current treatment plan, providers can verify the integrity of the medication list, identify discrepancies, and ensure appropriate follow-up.
