AXI-123 recovery audit — fully superseded; do not merge

PR #64 preserves the original recovery commit 9d028003479033bfc4abe9962949ce7b645ff1d4. AXI-123 audited the complete snapshot against current origin/main.

One-time branch exception

The operator explicitly authorized retaining the legacy branch codex/recovery-axi-102-human-action-visibility solely to preserve this primary PR and its review/provenance history. AXI-123 remains the sole Forge Delivery owner. This branch must not be renamed or duplicated, and this PR must not be replaced.

Provenance result

The recovered implementation had already landed through merged PR #21 as squash commit a1b702ab9e491d32f922121e5713b36bf98ab5c4, which is an ancestor of current origin/main.

Eighteen recovered code, test, changelog, and package-version files are byte-identical between the recovery snapshot and a1b702a. The only differing file is DEVLOG.md.

Classification

Still-valid behavior, but already present on current main (therefore superseded as recovery work):

  • first-class Command Center runtime approvals, decision counts, and inline approve/reject controls
  • direct issue-bound ActionRequest cards and approval UI cache refresh
  • issue run resolution independent of mutable agent assignment, with approval-first ordering
  • deduplicated atomic approval capture/resolution across poller, subscription, and operator races
  • durable run events plus audited/realtime blocked activity carrying issue context
  • approval attention de-duplication across agent surfaces
  • patient WAITING runs excluded from generic stale-run abandonment
  • focused router/service regression coverage for all of the above

Unsafe or obsolete to restore:

  • the historical v0.10.1 package/changelog release state
  • the recovery DEVLOG wording that replaces PR fix: surface human-action stalls across related views #21's completed full verification with “verification in progress”
  • unrelated old chat-runtime DEVLOG history carried by the recovery branch base
  • any raw replay of the v0.10-era files, which would regress the later lifecycle, provenance, Command Center, runtime-expiry, patient-wait, and Delivery/GitHub truth changes now on main

Outcome

No cohesive missing change survives. No source, test, docs, DEVLOG, version, or changelog diff was manufactured. The PR was not made ready, linked as IMPLEMENTS, attached as an implementation, reviewed, or merged because it delivers no missing behavior. It is closed unmerged as fully superseded.

No release, tag, deploy, production checkout, production database, or production service was touched.

AXI-123 audit complete: the functional snapshot is byte-identical to code already merged in PR #21, while the only remaining DEVLOG delta is unsafe historical noise. Closing this recovery PR unmerged; the grandfathered branch and recovery commit remain intact for provenance.
