Summary

• keep every open ActionRequest independently visible in Command Center, even when several target one issue
• give legacy reply-less recovery asks safe Dismiss and Open issue actions
• reconcile typed and historical delivery conflicts when the blocked candidate is terminal or the Delivery session ends
• preserve quiet MCP ownership until the exact connection resumes or an explicit lifecycle transition releases it

Root cause

Command Center collapsed all open requests to one survivor per issue, while legacy FREE_FORM recovery requests without a requester exposed no resolving action. Work-session cleanup also ignored execution-conflict dedupe keys, so terminal blocked dispatches could remain open indefinitely.
