Seen 2026-09-28 on NP02J (K68), runtime v2.21.11, in-process vk_native, Avatar demo (zones app: 3D zone + Local2D, draw-based alpha-over pass) built from avatar#113 (_SRGB swapchain).

Each failure drops a frame; the user sees an occasional flicker. -3 is VK_ERROR_INITIALIZATION_FAILED, an unusual result for a queue submit — worth checking whether it comes from a wrapper/fence path rather than vkQueueSubmit itself.

Not yet known: whether it predates the #1589 format-honest compose target (i.e. also happens with the released UNORM avatar v0.14.0 / DXR_COLOR_LEGACY_UNORM_ENCODED=1), and whether other zones apps hit it. Separately, Avatar's Android background capture / transparent-atlas fringes at de-occlusions remain open.
