Fixes #182.

The README still presented feed subscriptions as what the bot is. Every command now names its source, and INTERNAL_PLUGINS plus the plugin catalog are documented as the host's real surface.

Two deviations from the issue text, both because the issue predates the rename: it says CORE_PLUGINS, the code says INTERNAL_PLUGINS; and it frames settings as a plugin command, but /settings is a host command whose panels come from plugin manifests.

Not included: docs/architecture.md:36-38 still names /welcome and /voice-settings commands the manifests do not declare. Filed as #199 rather than widening this diff.

The README still presented feed subscriptions as what the bot is, which
stopped being true when the features moved into plugins. It was the
loudest remaining trace of the old architecture.

Every command now names its source: /feed and /vc from their plugins,
/settings /plugins /about from the host, with settings panels coming
from plugin manifests. INTERNAL_PLUGINS and the plugin catalog are
documented as the host's real surface.

Uses INTERNAL_PLUGINS, not the CORE_PLUGINS name in #182's text; the
rename landed before this. settings is described as a host command
because that is what it is.
