docs: define MCP remote operations preflight

Outcome

Define the implementation work order for making remote 0102 operational through MCP before root main.py continues.

Scope

• Requires a real MCP initialize/list-tools/tool-call handshake.
• Makes non-empty generation-bound HoloIndex semantic retrieval a hard gate.
• Separates repository-owned MCP services from MCP-host-owned GitHub/web connectors.
• Defines topology recovery, process/auth lifecycle, operational receipts, tests, and merge evidence.
• Explicitly excludes FoundUp lifecycle, pfMALL, 3V/economics, simulator, DAEs, Selenium, and generic launch dependency auditing.

Truth boundary

This PR is documentation only. It does not claim the MCP bootstrap is implemented or that remote 0102 is currently operational.
