refactor(ui): unify action buttons, modal close triggers, and delete components

• Portal Consistency: Refactored modals, dialogs, lists, and table action buttons across \�pps/portal\ to use semantic theme tokens and centralized delete/close components.
• Config & Integrations UX: Streamlined \AppConfigSelectorModal\ and standardized action buttons across integrations, auth cards, and project settings.

Testing & Verification

• Lint and typecheck verified across packages (\�pps/portal, \�pps/server, \packages/components).
• Pre-commit test suite executed and passed.
