Rework paths use ItcyTools in-memory handles after /handle_add. Rename VS Code entry, skip short-name false positives on "code reviews", match DoorDash brand labels. Brave lock waits up to 180s and clears stale PID files. Co-authored-by: Cursor <cursoragent@cursor.com>
