Same-day follow-up to #136: doc-contract fixes for all seven friction findings from the first live upgrade-recap run (acredix.cl pilot). Finding→fix table and harness evidence: dev/seo-growth-workspace/2026-07-15-v5.2.1-release-run.md.

  • Report home fixed (reports/YYYY-MM-DD-upgrade-recap.md); stamp records it.
  • Stale mechanism named (staleAsOf/staleReason + backlog-row exit) and made certificate-binding in § Coverage certification; reader unchanged.
  • No retroactive recap ticket on direct operator invocation.
  • Re-triage scoped to the version window with version-driven / incidental hygiene outcome classes.
  • Load-bearing invalidation test for coverage rows.
  • Template gains the Upgrade context section; Operator column is authorized-by / executed-by.

Harness: validator 52/52, inventory clean, evaluate-release 100/100 (gate-results-5.2.1.json). No script behavior changes; no ratified decision from map #127 reopened.

…p-after-recap must not overwrite history

Autoreview P1: a fixed daily filename would have overwritten acredix.cl's
5.2.0 recap report during today's 5.2.1 recap. Report path is now
reports/YYYY-MM-DD-upgrade-recap-<installed version>.md in contract and
template. Gate artifact rebound; harness green.

Same-version same-day reruns (e.g. after a malformed stamp) now append
the first free numeric suffix instead of overwriting; the stamp records
the path actually written. The report header/context fields gain
explicit never/malformed drift states with the conservative baseline
rule. Gate artifact rebound; harness green.

Autoreview disposition (merge evidence): Codex ran three passes over two fix rounds — round 1 caught the same-day recap-after-recap filename collision (P1, would have overwritten acredix.cl's 5.2.0 report today), round 2 caught same-version rerun collisions and the unrepresentable malformed-stamp state. All fixed. Final pass on e8ced19: clean, "patch is correct". Harness green at the same head (52/52, inventory clean, evaluate-release 100/100, gate-results-5.2.1.json digest-bound). Merging per Jorge's instruction for this release.
