P1: README and release docs sync for support/upgrade/npm publish

Problem:
README command list and release instructions are out of date (missing upgrade/support commands and npm publish automation).

Acceptance Criteria:

• README command table reflects current CLI commands (include upgrade/support/paths/repair/check as appropriate).
• Release section documents workflow_dispatch reruns and npm publish trigger.
• Docs remain consistent with generated command reference.

Test Plan:

• npm ci
• npm run lint
• npm test
• npm run build

Plan:

• Update README command table to include upgrade/support/paths/repair/check/ps1 where applicable.
• Update Release section to mention workflow_dispatch reruns and npm publish on release.
• Verify docs/commands.md stays in sync (run docs:generate if needed).

Risks:

• Minimal; documentation-only changes.
