Improve PersianBlocker README

Summary

• Replaces the current link-heavy README with a clearer Persian project overview.
• Adds quick setup instructions for uBlock Origin, AdGuard, and manual filter subscriptions.
• Adds a table explaining the main filter files and their raw subscription links.
• Documents reporting, contribution, and local development workflows.
• Keeps a respectful note for Sayyed Ali "Master" Kia, the original author and maintainer.

Why

The existing README is difficult to scan and mixes project documentation with many external references. This update makes the repository landing page more useful for new users, contributors, and maintainers by putting installation, file selection, and support information first.
