Rebuild checkpoint review packets for every revision - #119
Every routine/integration reviewer spawn now rebuilds its packet from an explicit live-source recipe. Composite artifacts, state and the current log slice are regenerated together; hashes are computed from those source bytes. Before dispatch, the exact packet and slice are archived under the spawn ID outside the reviewer's read root. Reused spawn IDs and staged/archive sources are refused. Cursor resume validates the frozen read-set instead of rebuilding it mid-Task. Derived log-slice names are blocked in routine and readiness packets; recipes also reject full logs, transcripts, verdicts and archived slices as sources. Failed watcher recipe writes notify escalation before poison-marking.
The Delegate, Conductor and legacy watcher follow the new staging contract. Existing callers must provide checkpoints/NN-review-sources.json; missing recipes fail closed. Adapter fixture mocks were updated to supply the already-required Artifacts-read field.
Validation: nine new staging tests; Claude/Codex/Cursor packet contract checks; focused adapter, watcher and Cursor recovery fixtures, including the recipe-failure notification. An end-to-end repeated-review test verifies changed source bytes reach the second reviewer while the first archive remains unchanged. Shell syntax and diff checks pass. Local framework validation encountered the previously reproduced fixture #309 polling-timeout failure; GitHub CI is the merge gate. Full regression suite was not rerun.
Cold-review packets are rebuilt from live source recipes for each new reviewer spawn. The stager validates inputs, generates packet hashes, and archives the packet and log slice. Resume validates the frozen packet. Watcher and regression fixtures now provide source recipes and artifact-read hashes.

The protocol and delegate instructions describe rebuilding packets from live sources. The watcher writes source recipes and stores integration gate evidence separately. The instructions restrict log-slice filenames, and the watcher fixtures include artifact-read hashes.
Build and archive each reviewer packet scripts/stage-cold-review.py, scripts/check-cold-review-staging.py, .bureau/regression/332-cold-review-source-freshness.md, .bureau/regression/229-codex-cold-reviewer-adapter.md, .bureau/regression/296-cursor-cold-reviewer-resume.md
The stager validates source recipes, builds packets from current bytes, and archives each spawn’s packet and log slice. Tests cover refresh, resume, archive matching, and rejected inputs. Regression fixtures provide source recipes and artifact-read hashes.
Runner integration and reviewer fixtures scripts/run-cold-reviewer.sh, scripts/check-cold-reviewer-packet.sh
The runner invokes staging for new spawns and frozen-packet validation for resume. Packet generation and manifest creation move to the stager, and the packet regression script checks the new resume error and runs the staging tests.

Full details: Title check
Explanation
The title "Rebuild checkpoint review packets for every revision" matches the main change. The diff rebuilds cold-review packets from live sources on each spawn and archives per-spawn evidence, which addresses the re-return staleness in issue #98. The title is short, specific, and uses no vague terms. It does not name the enforcement method or the new gate-sources separation, but the PR does not need to cover every detail.
Full details: Linked Issues check
Explanation
Issue [#98] requires fresh staging on each re-return, rebuilt composite/state/digest content, protocol guidance, enforcement, and regression coverage. The reviewed changes document live-source rebuilding for every spawn, external ordered source recipes, refusal of reused or staged/archive sources, exact per-spawn packet and log-slice archives, and frozen-packet validation on Cursor resume. scripts/stage-cold-review.py hashes source-derived bytes and publishes only after source stability checks. scripts/check-cold-review-staging.py covers revised sources, stale-file removal, archive isolation, resume validation, reused spawns, unsafe sources, and derived log-slice names. The Delegate and watcher staging contracts and the v2 protocol documentation were updated.
Full details: Out of Scope Changes check
Explanation
The changes remain connected to issue [#98]. Delegate, Conductor, watcher, adapter fixtures, and packet-contract changes implement or validate the new staging contract across supported review paths. Integration evidence isolation, per-spawn archives, source safety checks, and derived log-slice restrictions prevent stale or contaminated review packets. No demonstrated unrelated change appears in the reviewed scope.
Full details: Docstring Coverage
Explanation
Docstring coverage is 10.00% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 10 functions across 5 files. (8 skipped: 8 unsupported.)
Resolution
Write docstrings for the functions missing them to satisfy the coverage threshold.

agents/delegate.md:335-337 🎯 Functional Correctness | 🟡 Minor | 💤 Low value
Stale CI files stay in gate-output/ after a local re-run.
integration-gate.sh creates gate-output/ with mkdir -p and never removes it. CI mode writes ci-checks.json and ci-poll.log into that directory. A local re-run of the same checkpoint writes only its own files, so the CI files remain. The bridge recipe says to include each retained gate-output/ file, so the stale CI files are part of the staged source set. The reviewer prompt names only the manifest files and integration-results.json, so the reviewer is not directed to read these files.
Note: this behaviour predates this PR (already present at merge base 26468da17dfd); the change keeps it reachable.

Update integration-gate.sh to remove the existing gate-output directory before
recreating it for each run, so files from a previous CI run cannot enter a local
checkpoint’s staged source set.
