Summary

add root-level fly.toml so Fly GitHub integration can detect app config
add root Dockerfile.fly that builds and runs interview-app
update deploy workflow to use --config fly.toml
update docs and docker ignore for root build context

Validation

cd interview-app && npm run check
cd interview-app && npm test
