First db->pdf transform version

This is the first version of the db->pdf transformation ready for testing.

Bypassed chromatic test to merge - chromatic failed due to missing token. Pull-request test, prettier + linter etc., passed

Some issues may remain to be resolved, but testing and demo should be possible regardless.

Possible Issues remaining:

• Disclosure form:
  • The 'disclose to' PDF-fields don't have checkboxes, only text fields. Paper form has checkboxes and text fields
  • The 'disclose to' database fields only have checkboxes, no text fields. Paper form has both.
  • Missing field in PDF for disclosure of information about 'Medical/Psychiatric Evaluation/Treatment Records'
• Healthy Marriage and Responsible Fatherhood Assessment Worksheet:
  • Checlbox-field for 'Financially eligible according to the appropriate income/resource standards' is missing from PDF form
  • Checbox-field for 'Pay Stub' is missing from PDF form
• Healty Marriage and Responsible Fatherhood Introductory Survey
  • Checkbox-fields for 'Why are you here?', 'How did you hear about this program?' and 'What do you expect to gain from this program?'`are multiple choice on paper form and in database, while they are single option in the PDF-form
  • Option for 'Help Optaining Information About Health/Wellness' missing from database
• Individual Service Plan
  • No fields for 'Service Areas' in PDF, the box for 'Domestic violence' seems to be checkbox on paper form, multiple choice field in database

Awesome! Nice and thorough. Thank you for the comments on the fields that we need to check on. I'm good with getting this merged in. We can check with the team, Good Dads, and Neon on those remaining fields and use a separate PR to address those so that we can keep moving forward for the demo.
