docs: mark RFMS status evidence historical

Summary

• replace stale live-status language with a dated historical evidence snapshot
• preserve the June 17 command results and their exact local commit/environment scope
• record that the later preservation commit has no exact-tree rerun evidence
• expand explicit non-claims for CI, deployment, physical-device, pilot, and live-simulator validation
• refresh only the transitive ip-address lock entry from 10.2.0 to patched 10.5.0 within express-rate-limit's existing ^10.2.0 constraint
